Open Question: How do I stop my ipod tape deck adapter from switching sides and ejecting?

I have a monster tape deck adapter for my car & randomly, the auto reverse on the tape deck will switch sides repeatedly and eject the adapter. I still have tapes and, I have played them flawlessly in the deck so, that's not the problem. I have also replaced the adapter because, the last one did the same thing and I used a head cleaner on the deck. Please help me or recommend a brand that works
